HEALTH HOME TOOLS

The Washington Health Foundation developed a series of Health Home tools from 2009 to 2012.  These were an extension of the Foundation’s Healthiest State in the Nation Campaign, and the key strategic initiative within the Campaign to help develop Health Homes and People Centered Health Care.

These tools were developed by Foundation staff and made available through its website to interested persons.  Patients and consumers were encouraged to test the forms for their ability to help them control their health care, and the Foundation welcomed providers and others who wanted to explore the creative space of Health Homes.

These Tools remain relevant today and are being made available through the Foundation’s new website.  An overview of Health Homes and how the Foundation was thinking about these Tools can be found by clicking here.

The Foundation’s most prominent Tool was the “Just What the Patient Ordered: A Guide for Managing Your Hospital Care”.  Its use was for patients and families facing an inpatient hospital admission and stay, and provided advice and tools to help take greater control over the planned stay.  An English version of the Tool is available by clicking here.  A Spanish language version of the Tool is available by clicking here.

Another early Health Home Tool that was extensively used was “Rx For My Physician: A Tool to Help Manage Your Doctor Visits”.  This simple tool provided a way for patients to fill out and bring a document to physician and clinic office visits that would set their expectations and needs from the visit.  This Tool is available by clicking here.

Another tested and important Tool is “The Best Defense is a Good Offense: A Tool to Help You Manage & Maintain Your Health with Preventive Care”.   Developed to further the Foundation’s effort to dramatically improve Washington State’s state rank for the use of proven preventive clinical care, it guides users to key health actions.  This Tool is Available by clicking here.

Based on the success and reception to these Tools, WHF staff built a complete suite of Health Home Tools.  Many of these were not as well-tested as the three Tools noted above, but are being made available to interested parties on this website.  These can be accessed by clicking the Tools noted below.  Please let us know what you think of these Tools as you download and use them!
